Admissions

Our facility accepts referrals 24 hours a day, 7 days a week from professional referral sources, family members and patients alike. Preliminary information can be taken over the phone, in person, or by fax. Appointments may be scheduled but walk-ins are always welcome. All patients will be seen by our qualified team of registered nurses and/or master’s level clinicians.

Approved items for admission:

• Three complete outfits(please remove all strings)
• Two sleep outfits
• Three pairs of underwear
• Two bras (no underwire)
• One pair of shoes (please remove laces)
• No glass containers
• No alcohol in products
• Current prescription medications

Please bring the following documentation:

• Drivers license/Identification Card
• Insurance card
• Social Security Card
• Guardianship documents
• Custody papers (if applicable)
